ID Cards:

Your new Medical ID card has been requested and should have arrived by mail. If you have not received it you may request one by phoning Customer Service at the number above.

IMPORTANT: If you have Dental Insurance you ID card is in your WDS brochure enclosed.

RX:

Your new prescription drug coverage is effective in conjunction with your medical coverage. You may go online to www.regencerx.com to learn more about the prescription services available to you and to obtain a complete list of all the pharmacies that are on the preferred list.

Health Care Material:

Health care material such as claim forms, new brochures, application to add a new dependent and more is available through your human resources department and or health plan administrator. Please inquire on these items through them.

When selecting a physician for you and your families general needs consider a Family Physician board certified through the American Academy of Family Physicians.
